Safety Incidents - Multiple "Cause of Incident"

We are implementing Safe Work with one of our customers and their HSE team pointed out a valid point in the safety incidents. An incident could have many causes of why it happened so there is a need for additional boxes and the term “Cause of Incident” should be changed to something like ” Root Cause” which is more relevant and commonly used in Health and Safety industries.

My Proposal is the following,
